This is a case report of a seventy year old male with a swelling in the lower back.A Fine needle aspiration cytology conducted on the sample diagnosed the case as spindle cell sarcoma. Wide excision sample was sent to histopathology that showed predominant adipocytes with few pleomorphic bizarre cells, floret like giant cells and ropy collagen bundles. Re-evaluation of cytological smears dawned on us authors the pitfalls in the diagnosis of Pleomorhic lipoma.We present this rare case in view of minimizing radical surgeries in such deceptive benign lesions.
