
Takotsubo Cardiomyopathy

A 46-year-old woman presented with chest pain. EKG showed no acute changes. Troponin was initially normal, but significantly increased in 3 consecutive measurements. Angiogram was negative for coronary artery disease. Ventriculogram showed hypokinesia of the inferior wall and ballooning of the left ventricle (Figure and Video). Patient was discharged home in stable condition after 2 days of medical management. Echocardiogram was normal on the day of discharge. Apical left ventricular ballooning with preserved function of the base was first described by Sato et al in 1990. In Japanese, “takotsubo” means “fishing pot for trapping octopus,” and the left ventricle of a patient diagnosed with this condition resembles that shape. Also known as “broken heart syndrome”, this condition is more commonly seen in post-menopausal women with a history of emotional distress. There is no confirmed evidence of a genetic predisposition to date. Treatment is generally supportive in nature.
